Specimen Requirements

Container/Tube:

Preferred: Green-top (sodium or lithium heparin gel)

Acceptable: Gold-top serum gel tube, plain red-top tube

Specimen Volume: 1.875 µL plus required dead volume of plasma or serum

Collection Instructions: Peak level occur 30 to 60 minutes following an oral dose.

Note: 1. Time to steady state is 5 to 20 hours.

2. Severe liver damage can occur at levels >150 mcg/mL at 4 hours post ingestion, or 50 mcg/mL after 12 hours. Antidotes are available but need to be administered within 8 to 10 hours after overdose.

Reference Values

Therapeutic range: 10-20 mcg/mL

Critical value: ≥150 mcg/mL

Acetaminophen (brand name: Tylenol®) levels are measured to determine concentrations in the blood stream.  This can be of extreme importance in cases of suspected intoxication because effective antidotes are available.
