Specimen Requirements
Container/Tube:
Preferred: Green-top (sodium or lithium heparin gel)
Acceptable: Gold-top serum gel tube or plain red-top tube
Specimen Volume: 0.2 mL of plasma or serum
Stability: Acetaminophen- 2 hours
Salicylate- Separated specimens should be analyzed immediately or kept at
room temperature for 7 days or 2 – 8 °C and analyzed within 2 weeks.
For longer storage, samples may be frozen at -20 °C for up to 6 months.
Collection Instructions: Acetaminophen- Peak levels occur within 30-60 minutes after an oral dose .
Salicylate- Peak levels occur 1 – 2 hours after a dose, depending on the drug
formulation.
Reference Values
Acetaminophen- Therapeutic range: 10-20 mcg/mL
Critical Value: ≥ 150.0 mcg/mL
Salicylate- Analgesic therapeutic range: 2-10 mg/dL
Antiarthritic therapeutic range: 15-30 mg/dL
Critical Value: >30 mg/dL

Acetaminophen (brand name: Tylenol®) levels are measured to determine concentrations in the blood stream. This can be of extreme importance in cases of suspected intoxication because effective antidotes are available.
Salicylate measurements are used in the diagnosis and treatment of salicylate overdose and in monitoring salicylate levels to insure appropriate therapy.
