Test Code PBNP Pro B-Type Natriuretic Peptide, LAB2670
Performing Laboratory
St. Luke's Hospital Cedar Rapids
Specimen Requirements
Preferred Container/Tube: Mint green-top (heparin) tube
Acceptable Container/Tube: Green-top (heparin) tube, lavender-top (EDTA) tube, red-top or gold-top (SST) serum tube
Specimen Volume: 9 uL plus a dead space of plasma or serum
Stability :Centrifuge and separate the plasma or serum from the cells immediately or within 2 hours of collection for transport. Store separated samples at 6 days at 2-8ºC, and test within 3 days of collection. Sample is stable for 3 days at 20-25ºC and 24 months at -20ºC. Freeze only once.
Reference Values
<300 pg/mL
Under 50 years of age:
NT-proBNP values below 300 pg/mL have a 98% negative predictive value for excluding acute congestive heart failure (CHF). For adults under 50 years of age with intact renal function, NT-proBNP values greater than 450 pg/mL have a positive predictive value of 76% for the diagnosis of CHF. For patients with an eGFR below 60, a cutoff of 1,200 pg/mL yields a diagnostic sensitivity and specificity of 89% and 72% for acute CHF.
50 to 75 years of age:
NT-proBNP values below 300 pg/mL have a 98% negative predictive value for excluding acute CHF. For adults 50 to 75 years of age with intact renal function, NT-proBNP values greater than 900 pg/mL have a positive predictive value of 83% for the diagnosis of CHF. For patients with an eGFR below 60, a cutoff of 1,200 pg/mL yields a diagnostic sensitivity and specificity of 89% and 72% for acute CHF.
Over 75 years of age:
NT-proBNP values below 300 pg/mL have a 98% negative predictive value for excluding acute CHF. For adults over 75 years of age with intact renal function, NT-proBNP values greater than 1800 pg/mL have a positive predictive value of 92% for the diagnosis of CHF. For patients with an eGFR below 60, a cutoff of 1,200 pg/mL yields a diagnostic sensitivity and specificity of 89% and 72% for acute CHF.
